How long does prescribed medical cannabis stay in your system?

There is no reliable personal “clear by” time. THC or its metabolites may remain detectable after the noticeable effects have passed, and a negative or low result does not reliably prove that someone is fit to drive or work safely.

Detection and impairment are different questions

A test asks whether THC or a THC metabolite is present above the method’s reporting threshold. Impairment asks whether attention, judgement, coordination, reaction or another relevant ability is affected at that moment.

A positive result does not automatically measure current impairment. Equally, a low or negative concentration does not guarantee safe performance. Research consistently finds that biological THC concentrations are relatively poor individual predictors of driving impairment.

Why no single detection window is honest

The result depends on several variables that cannot be reduced to one number:

  • how frequently the medicine is used and how long treatment has continued;
  • the THC content, amount used and timing of the last administration;
  • whether the product was inhaled or taken orally;
  • individual metabolism, body composition and tolerance;
  • the specimen tested—blood, oral fluid, urine or hair;
  • the laboratory method, confirmation process and cut-off concentration.

Frequent high-dose users can have residual THC or metabolites for substantially longer than occasional users. A study of dependent users found THC detectable in some blood samples for at least seven days after stopping, illustrating why an individual prediction based on hours since last use can be misleading.

What different tests can and cannot show

Blood

Can identify and quantify active THC, but the concentration changes quickly after use and does not map neatly to individual impairment. Frequent users may have residual concentrations after the acute effects have passed.

Oral fluid or saliva

Often used for roadside or screening purposes because it is practical and can indicate relatively recent exposure. It remains a screening or detection tool—not a personal impairment meter.

Urine

Usually detects the inactive metabolite THC-COOH. It can remain positive well after intoxication or impairment and cannot establish the exact time of use or current fitness for a task.

Hair

May be used to examine longer-term exposure patterns, but interpretation is complex and it is not a measure of present impairment or precise recent use.

Does feeling normal mean I am legally clear to drive?

No. Feeling normal is not a laboratory result, and a laboratory result is not a complete measure of driving ability. In England, Scotland and Wales, cannabis has a specified blood limit of 2 micrograms per litre. Government guidance says it cannot translate a dose into a reliable blood concentration because people metabolise drugs differently.

A statutory medical defence may be available when the medicine was lawfully prescribed and taken as directed, but it does not protect impaired driving. Never use an online estimate, home urine test or elapsed-time rule as permission to drive.

What does a workplace drug test mean for a patient?

Workplace testing normally follows the employer’s policy, including declared prescribed medicines, screening cut-offs, confirmatory analysis and review of the result. A prescription can explain lawful use, but it does not automatically remove role-specific safety requirements or every policy consequence.

Before testing, ask how prescribed medicines are declared, who sees the medical information, whether an initial screen is confirmed by an accredited laboratory, and how the employer separates detection from evidence of impairment or misconduct.

Can I calculate clearance from my monthly prescription?

No. Total grams or millilitres supplied do not reveal the amount used at a particular time, absorption, blood concentration, impairment or future test result. Product strength alone cannot produce a dependable personal clearance time either.
